Nigerians with missing vaccination card need affidavit, police report

Nigerians with cases of missing Covid-19 vaccination cards are expected to produce court affidavits and police reports before they can get replacements.

This was contained in a document published by the National Primary Healthcare Development Agency.

Nigeria commenced the first phase  Covid-19 vaccination in March 2021.

Citizens who get vaccinated are given vaccination cards at point of vaccination. The card serves as proof of vaccination.

The Federal Government has already declared its intention to make vaccination mandatory for civil servants and others who may want to access government offices.

Such people will be required to present their vaccination cards at point of entry.

However, the NPHCDA in its directive, stated that citizens who lose their cards would have no choice but to produce court affidavits and police reports.

The directive read, “The client would need to visit the vaccination site where he or she took the first dose of Covid-19 vaccination with a police report, sworn affidavit detailing the following; first name, middle name, date of birth and telephone number used during vaccination.

“Others are vaccination ID (if available), date of first dose (if known), date of second dose (if taken) and photo of lost card (if available).

“Once this information is provided and the client’s record is found either on the physical record or the EMID portal, a new card will be issued without delay at the same vaccination site”.
